Our practice is a membership-based medical practice based on treating our members/patients the way we would want our family to be treated.
Our members support the practice by paying an annual membership fee of $300, which helps us provide services and benefits beyond what insurance covers.
An example of the services we provide that are different than most medical practices:
Scheduling:
-We make your life easier by allowing self scheduling through our online scheduling portal.
When necessary you can schedule same day appointments.
Appointments:
-Dr. H spends additional time with you when necessary-you will never feel rushed or not listened to and we will work with you to develop a personal treatment plan that works for you.
Access:
-Members can email us anytime with questions or concerns. We answer emails in the evenings, weekends and holidays as necessary.
Treatment:
-We help coordinate follow up care with our office in addition to any other medical provider involved in your health care.
-We provide refills on medications urgently as needed and if indicated.
-We will obtain any needed authorizations on medication or treatment from your insurance company when necessary to minimize your out of pocket expenses as much as possible.
Membership Fee:
*ALL PATIENTS* are requested to pay a yearly fee of $300 and sign a Membership Agreement that outlines our office policies and terms of membership.
Copy Medical Records:
Each patient is entitled to a single copy of their
medical record, upon written request, at no charge.
Additional copies would incur a nominal clerical charge
of 25 cents per page + a nominal clerical fee.
*We reserve the right to amend the Office Policies/Membership Agreement at any time.
